Long-term Personal Care Services
Provides hands-on assistance with bathing, dressing, meal preparation, grooming, chores, laundry, grocery shopping and other activities necessary to meet the individuals daily living needs, as well as provides in-home support in cases where the individual is unable to care for themselves.
Criteria for Services:
•Medicaid Recipient
•Age 65 or Older
•Age 21 or Older with Disability

Supervised Independent Living
Provides support services for individuals who live or desire to live independently in the community. Individuals are assisted with training in money management and self-direction.
Criteria for Services:
•Medicaid Recipient
•Age 18 or Older
•MR/DD Waiver Recipient

EPSDT Personal Care Services
EPSDT (Early Periodic Screening Diagnosis and Treatment) provides assistance with direct care activities such as bathing, dressing, feeding, meal preparation, household chores, etc.

Personal Care Attendant
We provide direct hands on assistance with bathing, dressing, meal preparation, grooming, household chores, laundry, grocery shopping and other activities necessary to meet the individual daily living needs.

Respite Services
Provides in-home support for the individuals who are unable to care for themselves in the absence of the primary caretaker.
Criteria for Services:
•Medicaid or Medicare Recipient
•Age 65 or Older
•Age 21 or Older with a Disability
